Why Zivian Health
Most collaborating-physician services stop at the introduction: you get a doctor's signature, a PDF agreement, and a monthly invoice. Zivian Health's pitch is that the introduction is the easy part — staying compliant is where NP- and PA-owned med spas actually get hurt. It is one of the few vendors in this category that pairs physician matching with real software: a state-by-state rules engine, agreement management, chart-review workflows, QA-meeting scheduling and exportable audit trails.
The company has institutional weight behind it. In August 2023 it closed an oversubscribed $3 million seed round led by Wireframe Ventures with Crosslink Capital and NextView Ventures, per BusinessWire, to expand the platform to medical enterprises, insurers and telehealth companies. Zivian reports more than 7,000 collaborations matched from a roster of 2,000+ vetted physicians across all 50 states — company figures, but consistent with the enterprise clients it names.
What stands out
Speed and structure. Zivian claims an average collaboration match time of 3.8 days, with most providers matched in five days or less — for a med spa waiting on a medical director to open its doors, that timeline is the whole ballgame. Once matched, the platform automates the state-mandated cadence: chart reviews, quality-assurance meetings, sign-offs and documentation, with the company claiming clients save roughly 80 hours of compliance labour a month. The exportable audit trail is the quiet killer feature; when a board of nursing or medical board asks for records, you produce them in minutes.
The fee posture is also cleaner than much of the category. Zivian states there are no upfront, onboarding, matching or rematching fees, with subscriptions cited from roughly $500 per month depending on state requirements — same-state physician rules, chart-review volume and specialist-collaborator mandates all move the number. That is quoted pricing rather than a published rate card, but the floor and the no-fee promises are stated publicly, which beats the pure black-box quoting common among rivals.
Watch-outs
Zivian is a digital-health generalist. Aesthetics and med spas are an explicitly supported vertical, but the platform equally serves telehealth, psychiatry and enterprise medical groups — you will not get the injector-market fluency of an aesthetics-dedicated firm, and its physicians are matched for compliance coverage, not for hands-on aesthetics mentorship. Pricing beyond the cited floor still requires a sales conversation. And the impressive numbers — 2,000+ physicians, 3.8-day matches, 80 hours saved — are all company-reported; no verifiable third-party review aggregates exist yet, so we have left the ratings field empty and suggest asking for med-spa-specific references during diligence.
Who it's for
Zivian fits NP- and PA-led med spas in restrictive states where the collaboration itself is a genuine regulatory burden — multiple chart-review requirements, mandated meetings, documentation that must survive an audit. It is especially strong for groups operating across several states, where its rules engine replaces a patchwork of consultants. Solo operators in friendly states who just need a signature at the lowest cost may find leaner services adequate, and owners who want a medical director steeped in aesthetics culture should weigh a specialist instead. But for compliance-serious operators, Zivian's software-plus-matching model is the most complete package in this review set.
